Question 1: Is the Check Engine Light On?
If yes, scan first. The stored codes often point directly to the cause. Misfire codes (P0300, P0301-P0312), lean codes (P0171, P0174), MAF codes (P0101), and others give specific direction. If the light is not on, we work through the categories more systematically.
Cause 1: Vacuum Leak
The engine needs a specific air-fuel ratio at idle. A vacuum leak introduces unmeasured air, leaning the mixture. The engine compensates as best it can, but idle suffers. Common leak sources: intake manifold gasket, PCV hose, brake booster hose, intake boot.
Symptom pattern: rough idle, possibly with a slight hiss from underhood, fuel trims showing lean (visible on scan tool).
Fix: locate and repair. Most vacuum leaks are $80 to $340 to fix; intake manifold gasket can be $480 to $900.
Cause 2: Misfire (Spark Plug or Coil)
A cylinder not firing causes obvious rough idle. Modern coil-on-plug systems use individual coils. A failed coil or worn plug causes a single cylinder to miss.
Diagnostic: misfire codes identify the cylinder. Coil swap test confirms.

Cause 3: Dirty Throttle Body

Fix: throttle body cleaning, $80 to $180. Often combined with throttle position relearn.
Cause 4: Carbon Buildup on Intake Valves (DI Engines)
Direct-injection engines develop heavy carbon deposits on the intake valves over 60,000 to 100,000 miles. This disrupts airflow and causes rough idle, misfires, and rough running.
Fix: walnut blasting of intake valves, $480 to $680. Required maintenance on many BMW, Audi, VW, Mercedes DI engines around 60,000 to 80,000 miles.
Cause 5: Failed MAF or MAP Sensor
The mass air flow or manifold absolute pressure sensor tells the engine computer how much air is entering. A failing sensor sends inaccurate data, causing the engine to run with the wrong fuel mixture.
Diagnostic: scan tool data, sensor signal comparison to known good values.
Fix: sensor cleaning or replacement, $80 to $280.
Cause 6: Failed Engine Mount
A torn engine mount lets engine vibration transfer to the body. The vibration is felt strongest at idle when the engine is in a single rotational state and is most sensitive to mount condition.

Frequently Asked Questions
Can a dirty air filter cause rough idle?
Rarely on its own. A severely clogged air filter affects acceleration more than idle. We check filter condition as part of any rough idle diagnosis but it is not usually the cause.
Should I try fuel injector cleaner before bringing it in?
Worth trying. Quality fuel injector cleaner (Techron, Sea Foam, BG 44K) can resolve mild rough idle from dirty injectors. If symptoms persist after a tank or two of treated fuel, get a diagnostic.
How long does throttle body cleaning take?
About 45 minutes typically. Includes cleaning the throttle plate and bore, sometimes a throttle relearn or adaptation reset. $80 to $180 depending on vehicle.
Is walnut blasting really needed on my BMW or Audi?
On direct-injection European engines, yes, usually every 60,000 to 80,000 miles. Carbon buildup on intake valves is unavoidable on these engines and walnut blasting is the most effective cleaning method.
